Case Management and ECHO
HNFS has physical and mental health case management programs for beneficiaries enrolled in ECHO who meet specific criteria.
Case management includes the development of a care plan to identify short-term and long-term goals with scheduled appointments with a case manager. Case management services end when established goals are met, the beneficiary changes regions or the beneficiary elects to discontinue case management services.
To request an evaluation for case management, complete the Nominate a Beneficiary for Case or Disease Management form (log in required). Select "case management" on the form.
